In Deutschland und in den westlichen Industrienationen zählt der Schlaganfall zu den häufigsten Erkrankungen und steht an dritter Stelle der Todesursachen. Bis heute fehlen im Gesundheitswesen fallbezogene Ansprechpartner, die Patienten und Angehörige nach dem Schlaganfall begleiten sowie beratend und koordinierend zur Seite stehen.
In dem vom Innovationsfonds geförderten großen Schlaganfall-Lotsen-Projekt STROKE OWL hat die Stiftung Deutsche Schlaganfall-Hilfe ein neues Versorgungsmodell für Schlaganfall-Patienten entwickelt. Die Schlaganfall-Lotsen arbeiten nach einem strukturierten Care und Case Management-Prozess und sind verlässliche Ansprechpersonen für die Betroffenen.
Das „Handbuch Schlaganfall-Lotsen“ fasst erstmals die vielfältigen Erfahrungen, die in der vierjährigen Laufzeit des STROKE OWL-Projekts gesammelt wurden, in kompakter Form zusammen. Die Arbeitsweise der Lotsen, ihre Aufgaben und Qualifikation werden darin ausführlich beschrieben.
